All individuals applying for certification as an Independent Provider and the Chief Executive Officer (CEO) of an
agency certified by the Ohio Department of Developmental Disabilities (DODD) are required to have a criminal
background check conducted by the Bureau of Criminal Identification and Investigation (BCI). If you have lived in
Ohio for at least five years, you are required to have only an Ohio criminal background check. If you have lived in
Ohio for fewer than five years, or if you were convicted of a crime in another state or of a federal offense, you must
request both an Ohio background check and a FBI background check.

Answer (A). No you cannot. The VA agency that is processing your request for background investigation must FIRST initiate an eQIP account for you. After your eQIP account has been initiated, you will receive an email notification from the Office of Personnel Management (OPM) containing your eQIP Registration Code (14 characters). You can then access the eQIP website and register. The link to eQIP is: http://www.opm.gov/e-qip/
