General Requirements
An original and a photocopy of the application form (2pages)
An original and two (2) photocopies of Official (Original) Transcript of Records
A Certified True Copy and One (1) photocopy of Diploma with the university colleague seal and signature of the registrar in ink
Two (2) copies of Curriculum Vitae
Two (2) complete and signed recommendations (forms provided in the Application Packet). The sealed envelope must be addressed to: THE DIRECTOR, NGOHS
Photocopy of Birth Certificate
Photocopy Maariage Contract for married female applicants
An Essay on an 8 1/2 x 11" sheet of paper describing your motivation for pursuing graduate study and your view of self-directed learning as a method of instruction likewise, provide a description of your research interest
A photocopy of PRC License/Certificate (for MRS.MRSSP, MS Dentistry, and MA in Nursing Applicant)
A dissertation proposal abstract and published creative works (for PhD Nursing applicant)
An original and one (1) photocopy of official receipt of a applicant fee
Four (4) passport-size pictures
MHPEd/MAHPEd Requirements
A 4-year baccalaureate degree in health professions from the University of the Philippines or other approved university or tertiary institution considered acceptable by UPM
Teaching and/or administrative experience or at least two years in an institution responsible for the teaching of health personnel
Good scholastic record from any recognized institution of higher learning
For all applicants from countries where English in not medium of instruction Certificate from the International English Language Testing System (IELTS)(i.e at least SS0 for written test, 173 for computerized test, and 70 for internet-based test)
Duly accomplished application forms (available for download at http://ngohs.upm.edu.ph/ with the minimum National Graduate Office for the Health Sciences (NGOHS) requirements
DrHPEd Requirements
Be a Master's degree holder in any health science or related field, from a recognized institution, Applicants with the Doctor of Medicine degree with valid professional license can be admitted directly to the program by virtue of CHED Resolution 038-2001
Minimum of 3 years in a health professions education setting as a faculty member, administrator, researcher, consultant, clinician, training officer, or human resource complement copy of official appointments is required.
Copy of at least one published paper where applicant is the primary author or co-author in referred journals recognized by UP in the recent 5 years
Submit a capsule dissertation proposal not exceeding five pages
For applicants who are not graduates of MHPEd, MAHPEd or any higher education related Master's degree, satisfactory completion of prerequisite courses, namely HP201 and 221. Applicants can be admitted as non-degree students at NTTCHP until completion of said courses
