Setting, Purpose, & Individuals

  • Sections with a red star MUST be completed

  • District-select from the drop down menu

  • Evaluation Plan Date-use the date the REED was opened

Purpose

  • Select from check boxes

    • Initial/Initial due to revocation of services

      • Requires a referral name-type in a name

    • Re-Evaluation-required every 3 years or sooner when eligibility is being reconsidered

    • Termination of services-used to discontinue a service but not end eligibility

Knowledgeable Individuals

    • Include all members that will be evaluating

      • This will cause a REED to show on each member's dashboard in Illuminate

    • Parent/Guardian

      • To list more than one, select change

      • Select Yes/No to include/exclude

      • Click Submit

    • General Ed Teacher, District Representative, Special Education Teacher/Provider

      • Start to type the name to open a down down menu

      • MUST select from the menu

    • To add more individuals, use Role and Name at the bottom of the page

      • Select the individual's role from the menu

      • Start to type the person's name

        • MUST be selected from the menu

    • Click Save

    • To navigate to the next section, use blue words above the student demographic information
