Transition Plan Invitation Information

  • Select Yes/No if the student needs a transition plan

  • If Yes, ensure that a Permission Form to Invite Outside Agencies has been signed by parent (dated prior to sending invitation)

    • If No, type why agencies were NOT invited--Not Applicable/Not Needed meets compliance criteria

    • Students OVER 16 years old are REQUIRED to be invited to the IEP meeting and MUST be included in the salutation

      • It is RECOMMENDED that younger students are invited to IEP meetings if appropriate

    • Explain steps that were taken to include student input in the IEP if the student did not attend

  • Save each section

Transition Goal

  • The Transition Goal page for Post-Secondary Education/Training will open

    • Type the additional education/training

      • MUST have current Casey Life Skills/assessment

      • MUST have score and date

      • Transition Services/Activities-LINK TO PDF

    • Type the goal (LINK goal to PDF)

  • Select Next

Post Secondary Education/Training

  • Select Yes/No if there is a need for an activity in this area (Post Secondary Education/Training)

    • Yes-opens a page to list a specific activity

      • Select the area from the drop down menu

      • Type the specific activity

      • Type the person/agency responsible

      • Select the date for completion from the calendar

      • Select Save

      • Select Yes/No to add more activities or move to the next section

    • No-opens Preferences/Interest and Goals (Employment)

      • Type the student's employment interest

        • MUST have current Casey Life Skills/assessment

        • MUST have score and date

        • Transition Services-LINK TO PDF

      • Type the goal

    • Select next

  • Select Yes/No to add activities for the area

    • Yes-opens a page to list a specific activity

      • Select the area from the drop down menu

      • Type the specific activity

      • Type the person/agency responsible

      • Select the date for completion from the calendar

      • Select Save

      • Select Yes/No to add more activities or move to the next section

    • No-opens Preferences/Interest and Goals (Adult Living/Community Participation)

      • Type student's living preference

      • Type goal for living preference

      • Select Save

    • Review Transition areas and activities

    • Select Next

Course of Study

    • Select the student's course of study from the drop down menu

    • Select anticipated exit date from the calendar

    • Select Next

Student Rights

    • Check the box that applies to student regarding Student Rights

      • If applicable, type in the guardian

    • Select Next
