Measurable Goal Requirement Sheet
A goal focuses on a slightly broader skill that can be addressed by measuring progress on 2 or more specific objectives
A benchmark goal focuses on one specific skill and goal progress will be broken down into equal increments
Click Add Goal
All red starred sections are required
Select the title of the person responsible from the drop down menu for reporting progress
Select when the objective will be addressed from the drop down menu
Skill/Behavior/Condition-Type the goal in the box
Accuracy/Criteria-Type the goal accuracy with a label (e.g. 80% accuracy, 3/5 trials, etc.)
Timeframe/Date-Type when the goal is to be achieved (e.g. by the end of the IEP, on specific date, etc.)
Link goal to a standard by searching or using the drop down boxes to find specific standards
Click Save Standard
Click Save Goal
Goal-Click Add Short-Term Objective
Areas with red stars MUST be completed
Describe Short-Term Objective-Type in the box what skill the student will improve
Select Evaluation Procedure from the drop down menu
Criteria-Type the goal criteria (e.g. 80% accuracy, 4/5 trials, etc.)
Select how often progress will be reported from the drop down menu
Type in Baseline Value (what the student can currently achieve)
Type in Target Value
Click Save
2 short term objectives are REQUIRED
Benchmark Goal-Click Add Short-Term Objective
Areas with red stars MUST be completed
Select Evaluation Procedure from the drop down menu
Select how often progress will be reported from the drop down menu
Type in Baseline Value (what the student can currently achieve)
Type in Target Value
Type in the measurement of the values (e.g. % accuracy, out of 5 trials, etc.)
Type in the number of reporting periods
Click Save
