Meeting Question and Purpose

  • Select Yes/No if a meeting is required

Yes-meeting required

      • Click Next

      • Purpose-select the purpose of the amendment

      • Setting

        • Host Site-select from drop down menu

        • Meeting Time-type in time for the meeting

        • Meeting Date-select the meeting date from the calendar

        • Meeting Location-Type in the location of the meeting

        • Click Next

      • Participants-boxes with red stars are REQUIRED

        • 1 parent name appears automatically

          • To add another parent, click the larger box next to Parent/Guardian

          • Then select another name

          • If a name is missing, notify local staff. Demographic information has to be entered via the local software system

        • For all other participants, start to type the person's name

        • Required participants: Parent, District Representative, MET Representative, General Education Teacher, Special Education Teacher/Provider

          • The name HAS to be select from the drop-down menu

          • A title for each person has to be typed

        • For preschoolers, a general education teacher is NOT required UNLESS student attends a preschool

          • The service provider could be listed as the general education teacher OR

          • Not Applicable can be selected from the drop down list

      • Click Save

      • Click Next at the bottom of the page to complete the next section

      • Select Yes/No to print the invitation

No-Meeting NOT required

      • Click Next

      • Purpose-select the purpose of the amendment

      • Amendment Information

        • Host Site-select from drop down menu

        • Amendment Date-select the date the amendment was published from the calendar\

          • Parent MUST be notified prior to amendment being published

        • Click Next
