If itinerant services are needed (Social Work, Occupational Therapy, Physical Therapy, Speech Therapy, and/or Audiology), services should be added with collaboration with itinerant staff
ESY-services should be listed separately from services before/after summer
IEP services could be listed three times reflecting three consecutive timeframes that run without a break (services for IEP meeting date to summer; summer to first day of next school year; first day of school to next IEP due date)
OR
do an IEP Amendment before school gets out to change services for ESY through the summer, and then do another IEP Amendment when the new school year starts to get back to regular school year service times

For consultation, this could be 0 if service time above is written as monthly
There may be some weeks that consultation is not provided but monthly time is met other weeks

Actual time is an average amount of time for the service
