Self-Assessment

This form can be used weekly or monthly for a learning skills snapshot. Students can quickly self-assess their learning skills, review their learning for the week, provide an opportunity to connect with the teacher in a non-threatening way, and establish goals for the next week or month.

This form can be used toward the Progress Report writing time. Students can review their portfolio of work, their involvement in school, and their learning skills. The template also allows for learning skill goal-setting (strengths and next steps) for the Progress Report as well. Finally, a Start-Stop-Continue section allows students to comment on the momentum of the class so far.

This form can be used at the end of the Term prior to report card time. The teacher may even review this form periodically throughout the term to define many of the indicators used to assess learning skills. Students should be given more time to go through the list of indicators for each, and then a brief opportunity to select a SMART goal for one of the learning skills. Students should be able to do this without a template at this point; however, the Progress template could still be used for this section.
