Coordinate next steps for writing the IEP with other team members.
Complete the IEP and write a Prior Written Notice (PWN). Send these to parents as soon as possible, but no later than 10 calendar days after the meeting date. See Prior Written Notice page for guidance.
Ensure that the PWN and IEP are finalized in SpEd Forms and, if applicable, that the Behavior Support Plan (BSP) has been uploaded to SpEd Forms history.
Make sure the following forms have been updated and filed:
1. If applicable: Assistive Technology Consideration Form
2. If applicable: Extended School Year Form (if the student has qualified for ESY)
3. If applicable: Emergency Health/Transportation Information Located in student's SpEd forms under Other Forms and Logs (if the student is authorized to access special transportation)
Communicate with all necessary team members about changes and any accommodations or supports that need to be put in place. This could include: the student, parents/caregivers, general education teachers, related service providers, paraprofessionals, administrators, services coordinators, School Resource Officer, social worker, school-linked mental health worker, and school psychologist.
Update graphs in SpEd Forms.
Complete a SpEd Forms Data Change if parent address, disability, federal setting, transportation status, school, or evaluation status were changed.
