This area should tell the student's story including reasons for referral, information from any previous evaluations, outside information or screenings, student strengths, parents concerns, and any other important information from their teachers, parents, or service providers. Background information/File Review is required for all evaluations .
A child is referred for an evaluation by a referral source. The specific procedures vary, depending on the age of the child. Before a student is referred for a special education evaluation, the district must conduct and document at least two instructional strategies, alternatives, or interventions using a system of scientific, research-based instruction and intervention in academics or behavior, based on the pupil’s needs, while the pupil is in the regular education classroom.
When a parent (must be guardian, not grandmother, stepmother, boyfriend, etc. unless the person has educational rights) makes an official request (oral or written (34 C.F.R. § 300.301(b)) for referral to Special Education the district must respond in writing (PWN or ER-PWN) within a reasonable amount of time (typically 7-10 days). All requests should be considered on an individual basis. If you are unsure as to whether or not to move forward with the evaluation process after a parent or outside request it is best to contact your district's Special Education Director or DPS.

As part of an initial evaluation and as part of any reevaluation, the IEP Team and other qualified professionals, as appropriate, must review existing evaluation data on the child including—Evaluations and information provided by the parents of the child.
Generally, there is no requirement regarding the amount of parent input.
However, some disability categories (such as DCD) require specific information from the home for eligibility.
