Other Health Disabilities

Other Health Disabilities (OHD) includes a wide range of chronic or acute health conditions, including Attention Deficit/Hyperactivity Disorder (AD/HD), that can range from mild to severe. Medications, treatments, therapies and repeated hospitalizations can affect a student’s ability to learn and function at school. A student with such a condition may be considered for special education under the OHD category.

Parent

• Parent Interview: Gathers data on medical needs, knowledge of condition, independence at home, and homework. 

• Parent Interview - High Needs: Gathers information on strengths/interests, parent concerns, student communication and self-awareness, daily living skills in the home, and social engagement.

Teacher

• Organizational and Independent Work Skills/Motor Skills Checklist - Infant & Toddler: Gathers data on organizational, work, and motor skills. The teacher rates numerous behaviors based on frequency of observation.

• Organizational and Independent Work Skills/Motor Skills Checklist - Pre-K & Kindergarten: Gathers data on organizational, work, and motor skills. The teacher rates numerous behaviors based on frequency of observation.

• Organizational and Independent Work Skills/Motor Skills Checklist - Elementary: Gathers data on organizational, work, and motor skills. The teacher rates numerous behaviors based on frequency of observation.

Student

• Student Interview: Gathers information on school perception, family, extracurricular activities, mood, medical history, and social activity.

Parent

• Parent Interview: Gathers data on medical needs, knowledge of condition, independence at home, and homework.

• Parent Interview - High Needs: Gathers information on strengths/interests, parent concerns, student communication and self-awareness, daily living skills in the home, and social engagement.

Teacher

Systematic Observation Rating Form: The observer make a binary rating for various behaviors under the categories of attention, time management/planning, motor/access, and organization/materials.
