Recommended CDS Approach for VTE Prophylaxis
In addition to the CDS/QI worksheets being used within individual care facilities to support CDS/PI efforts, they can also be used by health systems, HIT vendors, specialty societies and others to convey practices found to be particularly successful for applying CDS to specific PI imperatives.
The Society of Hospital Medicine has created a detailed worksheet filled in with their recommended CDS/QI approach for VTE prophylaxis (formats: Excel PDF (11X17")). These recommendations are based on SHM's extensive experience helping hospitals achieve excellent rates of appropriate VTE prophylaxis for their patients. (Note: the worksheet is a March 2013 update of the draft SHM VTE Prophylaxis recommendations originally posted to this Collaborative website in July 2012).
In addition to helping hospitals, EHR and CDS vendors, and others improve their CDS/QI efforts for VTE prophylaxis specifically, this completed CDS/QI worksheet illustrates how such such worksheets can be used to develop and disseminate best practices for other targets - ideally through collaborations among providers, HIT vendors, QI experts and other key stakeholders.
