Approaching the CDS Intervention Configuration Worksheet
Populating the Worksheet
At the bottom of this page is a spreadsheet that has several CDS/QI configuration templates and samples. The first two tabs contain a table of contents of the spreadsheet and brief instructions for using the templates, respectively.

Step 1: Decide if you are working on an inpatient or outpatient performance improvement target and select the appropriate tab from the CDS configuration template spreadsheet attached below. [Click on arrow to right of file at the bottom of this page to download document.]
The template has seven tabs:
Step 2: Consider creating workflow flow charts to help you and your team to get oriented to target-focused workflows

Step 3: Complete the top part of the template for describing the targeted performance improvement objective and your current performance on the target.
This step provides an essential baseline against which you can measure progress and you implement performance improvement interventions. The details may evolve as the template is used with other local stakeholders. The template specifically asks for:
Target= [specify improvement target]
Performance= [record available data about care processes and outcomes pertinent to your target. Data will change over time, so note date.]
Step 4: Review the first three columns of the spreadsheet (A, B, and C) that feature patient-specific activities and population-specific activities to determine which steps correspond to your target.
Modify activities that don't correspond to your workflow.
Step 5: Note the related care activities, specific tasks, and settings (columns D, E, and F).
Step 6: Apply the CDS 5 Rights to each step in the workflow (columns H, I, J, K, and L).

Step 7: Note implementation considerations (column N).
Step 8: Document proposed enhancements (locally or by EHR vendor) (column O).
This documentation can facilitate internal stakeholder conversations about CDS goals and approach, help enhance CDS planning, and enable information sharing with other organizations that are implementing CDS interventions for the same target.
Step 9: Sharing Completed Templates
Members of the CDS/PI Collaborative are sharing their completed templates on a page in the private Collaborative website and discussing them in the private project discussion group.
