Reducing Readmissions

The Society of Hospital Medicine's IT/Quality Committee has an initiative underway where its members are using CDS/QI Worksheets (see here) to document, analyze, share and improve CDS-enabled readmission QI strategies in their respective hospitals.

The completed worksheets are posted on the Collaborative's private website, available only to Collaborative participants. (Click here to join.)

SHM IT/Quality Committee members are discussing their worksheets in a corresponding online discussion group. You must likewise be a Collaborative member to participate in these discussions.
