As mandated by the Florida High School Athletic Association (FHSAA), every Shorecrest athlete must be cleared to participate for practices and for all team competitions. The clearance process requires the following:
A completed FHSAA physical (EL2) form signed by the student’s doctor
Signed Insurance/Liability/Heat/Concussion/Cardiac Arrest (EL3) forms. (These forms are available to digitally sign in the Athletic Clearance system.)
In addition, Shorecrest requires the following forms to be completed by a parent and a student:
Signed Athletics Handbook Acknowledgement form (This form is available to digitally sign in the Athletic Clearance system.)
Shorecrest Athletics Waiver, Release and Indemnification Agreements for EACH SPORT
NEW FOR 2025-2026 - FHSAA requires that all athletes (not their parents) complete the following free, online courses:

Once the courses are completed, students must upload the completion certificate from each course to their athletic clearance account. The completion certificate must have the STUDENT’S name on it, not their parents.
All the forms mentioned above will be collected via the FHSAA Athletic Clearance Website. We do not accept paper athletic forms in order to expedite and streamline the clearance process.
All athletes must be cleared through the Athletic Clearance System 72 HOURS BEFORE tryouts or their first practice.
It is HIGHLY recommended that the Athletic Clearance Guide (linked below) be used to prevent mistakes and to ensure all information is included when completing an Athletic Clearance.
All student-athletes and their families are expected to understand what is expected of Shorecrest athletes.
