DOR Student Services Program
Did you know that there are counselors that can help you explore different jobs right now? Apply for this service that can help you now, and for future careers.
Eligibility:
You must be a student (you need to be enrolled in high school, college or other school education)
You are ages 16 to 21 years old
At least one of these situations is true:
You have a 504 Plan, or you’re eligible for one if you want one.
You have an IEP.
You have a disability.
Others think you have a disability.

your transition specialist (Find their contact information at the bottom of this page) or
directly to the Department of Rehabilitation (DOR) Student Services Team (Find their contact information on this page before you see the forms)

This form is the first step in applying to student services.
Documentation can be provided by your teacher or transition specialist signing the form as a "School Official" . IEP is not required to submit at this time.

Release of Information - for exchange of information between Sequoia Union High School District staff and DOR staff

This form is to give permission for the DOR staff to exchange information with school staff
When this form is signed, we are only allowed to exchange the information that has a check mark.
This information is necessary for DOR to make their services unique to your particular needs. It also allows school staff to help advocate on your behalf and to provide documents that you may have misplaced.
If you want to limit the information that is shared, you may make a note or cross off any information you do not want shared.
The Release to exchange information expires one year after signing.

Release of Information (to be completed by Adult Students 18 yrs old and older)
- for exchange of information between parents and DOR staff.

This form should only be completed by Adult Students (ages 18+)
This form allows adult students to give permission for DOR staff to exchange information with their parent.
If adult student wishes to allow more than one family member to help advocate on their behalf, then one form must be completed and signed for each family member.
