2. Absences

  1. Create emergency sub plans.

3. Inform the office manager of the absence

4. Complete substitute evaluation form (teachers)

5. If you are out sick for three consecutive days or more, a doctors note is required to return to work.

3. Purchasing: Complete or request a purchase order prior to making a purchase, registering for a class, or professional development activity. (This includes mileage and any related expenses.)

4. Complete a workman’s comp form staff/accident form as soon as an accident happens but within 72 hours.

6. Facilities:

  1. Complete a building use form to inform the office and building staff of the event.

  2. Complete a ticket in SchoolDude to request support from the custodial/maintenance staff. It is located on the bottom of the Hopkinton School District main webpage as a link.

  3. Be mindful of egress.

  4. Be careful in the use of extension cords.

  5. Be aware of the restrictions in posting student work.

  6. Please be especially careful with school keys.

7. Make sure doors are closed/locked when you leave the building.
