Draw something on thicker paper or cardboard.

Apply a line of hot glue to the line areas you would like to be raised. You may also fill in some areas, but be careful, it is hot and expensive.

Allow to cool.

Coverer the glue areas with a coat of white paint. Allow it to dry.

Paint the rest of the image as you see fit.
