The Ontario Student Record (OSR) is the official school record for a student for any publically-funded school that they attend in Ontario. Every school holds an OSR for each student enrolled at that school. The OSR contains achievement results, credits earned, diploma requirements completed and other information important to the education of the student. Students and their parents/guardians (if the student is under 18) may examine the contents of the OSR.
If a student transfers to another school in Ontario, their new school will send a request to transfer the OSR to their previous school. The OSR cannot be transferred until a student attends their first day of classes at their new school.
Both the Education Act and Freedom of Information Legislation protect these records. The Ontario Ministry of Education, under the authority of the Education Act in the province of Ontario, requires that each school maintain a record of basic information for each student registered in the school. This information will be used for the purposes of proper education and well-being of the student and for necessary statistical purposes. Personal information on this form is collected under the authority of the Education Act, R.S.O. 1990 c.E.2, as amended, and will be used for the OSR and for administrative purposes.
