In addition to completing the Online Registration for Returning Students, the Health Information form may also be completed and submitted electronically in Skyward. This form is optional.

You may complete this form if your child has a medical condition(s) or has relative health information that needs to be relayed to the School Nurse.

Please be aware that the information given on this form may be shared with appropriate school staff in order to have a better understanding of the health status of your child. It is the responsibility of the parent/guardian to inform the School Nurse of any changes in the student’s health or medical status.
