Absence note

You may use this form to submit an Absence Note for your student. The information you provide should be as complete as possible. Once you submit the form, the school will be notified to accept the form. Once the campus accepts the form, attendance will be updated in Skyward with the appropriate absence codes. Absence Reasons that require documentation must be attached to the form.

Note: All online forms can be accessed and completed in a web browser or Skyward Mobile App.

STEPS TO CREATE AN ABSENCE NOTE

Log into Skyward Family Access from a computer, tablet, phone using a web browser or Skyward Mobile App.

Click on the Absence Note tile.

This form has one section:

Absence Note
- Select Absence Date
- Select Reason for Absence
  - If you choose "Other" please provide "Other Reason"
- Absence Documentation
  - Documentation is required for any absence reason notated with an asterisk.
- Review Terms of Agreement (Electronic Signature is required to complete this step)
- Click Submit Form
