Online Forms
Through out the year, Online Forms are used to collect and update information. Each online form is a process made up of steps to ensure the information the District has on file is accurate and up to date. An example of an online form is the Annual Verification form for the new school year. A parent or guardian completes this form to verify information such as email addresses, phone numbers, and handbook acknowledgements. You can start a form and return to it later. Any information that has been saved or completed will be there when you return to the form. Each step in an online form displays on a unique screen.
If the reviewer denies the form due to incomplete information, you receive a notification about the issue, and must update the information and resubmit the form.
Online forms are to be completed in a browser on a computer, phone or tablet.
We recommend forms be completed in Safari or Google Chrome.
Online Forms are now available in the Skyward Mobile App.
Some forms do allow multiple submissions and may appear as "New" after a form has been submitted.
Some forms do not allow multiple submissions. These forms will no longer appear after they are submitted.
Once a form is submitted you can review the information and status of the submitted form.
