Health and well-being are extremely important, and Notre Dame offers health and wellness services, insurance, and many other services to our students, faculty, and staff. The University Health Services (UHS) website can provide you with links to all the services provided here at Notre Dame, as well as updates to keep members of our community informed.
As an FLTA, you must meet the same health requirements and complete the same health forms as a new student. This means that you must update your immunization records and also complete a medical history form. More information about these requirements is below.
Once you have your medical history and your immunization records, you must log into your Online Student Health Account and enter your medical history and immunization information. More about immunizations can be found below and on the New Student Requirements page for Notre Dame.
Note: You must activate your university ID (NDID) in order to submit the requirements.

University Health Services (UHS) - Notre' Dame's University Health services provides excellent, patient-centered, confidential college health care that meets the needs of contemporary Notre Dame students to minimize health-related interruptions in their academic pursuits
University Counseling Center (UCC) The UCC offers a safe, confidential, and non-judgmental space for students who want to adress a wide variety of concerns and issues. They support students to face difficulties they may encounter and empower them to make the most of opportunities at Notre Dame.
McDonald Center for Student Well-Being - The McDonald Center offers a variety of services to help you improve your quality of life and flourish in all that you do. Whether you want to attend a workshop, or need a place to relax, the Center offers ways for you to build skills that help you to thrive.
Center for Student Support and Care - The Center for Student Support and Care is dedicated to providing Notre Dame students with comprehensive, individualized care to ensure academic and social success both in and outside the classroom. Through consultation, advocacy, and resources, the Center works on behalf of all students to help them thrive at Notre Dame and beyond.
As an FLTA, you will be automatically enrolled in the University-sponsored health insurance plan. Your plan will be paid for by IIE and Notre Dame. Do not worry about paying for or opting out of the University-sponsored health insurance plan. IIE and Notre Dame will be paying for you to be insured through this plan, as Notre Dame does not accept the health insurance you are given through IIE.
UHS can help you to navigate the insurance process and how to use your medical coverage.
Notre Dame requires that you submit your immunization records. Please visit the UHS website to read about immunization requirements. Click on the Welcome Letter link at the top of that page. After reading that, your next step is to download the University of Notre Dame Immunization Form. This must be completed by a medical provider to demonstrate proof of your immunization history.
If you do not already have some of the required immunizations listed on the top of that form, we recommend that you start making appointments to get those as soon as possible. Once you receive your university ID number (NDID), you will be able to log into your Online Student Health Account and enter your medical history and immunizations. Please do not worry about the deadline listed on the UHS website; it is no problem if your immunization form is submitted after that date.
UHS requires some international students depending on their country of residence to repeat their tuberculosis (TB) vaccine upon arrival. This is mandated by the state of Indiana. For example, the UHS considers India, Ecuador, Tunisia, Kenya, and Brazil to have high incidences of TB. If you are from a country currently listed to have high incidences of TB, you should expect to receive an email from UHs to set up an appointment once you arrive on campus.
Once you receive your NDID, you must complete and submit a Medical History Form. This form is available in the Student Health Forms in your Student Health Portal.
The medical history form asks for your U.S. phone number. If you do not have one, then leave that section blank. You may update it with UHS when you get to campus.
An emergency contact is required on your medical form. You will receive an email with the contact information of the CSLC staff, including their cell phone numbers. Please put down the Director and Assistant Director of the CSLC as your primary contacts.
