Driver Registration Form

Please complete the Driver Registration Form if you are a current Sheffield Hallam University student who has access to a private vehicle that you will be using in connection with transportation in relation to Club Sport Activity. This includes but not limited to:

Travelling to a venue for training sessions

Travelling to home and away fixtures

Transporting equipment between sites
