Make sure you have Form Publisher installed on your computer. You can check by clicking on the puzzle piece in the top right corner of Google Forms. Once you click on the puzzle piece, make sure you see Form Publisher. (You have to open Google Forms to see this.)
If you do not have the puzzle piece or Form publisher is not listed, go to the three dots in the top right corner of Google Forms.
Then click Get add-ons. Form Publisher should come up. If not, you can search for it.
Then install the add-on.
Click on Form Publisher.
Click Get started.
Click the purple/blue button that says Configure Form Publisher.
It now asks if you have a template. You do. It is the Google Doc that you saved in step 2. So select the option that says Yes, Select in Google Drive.
It will then process everything and a purple/blue button will come up that says Try your form. You will need to do this one time before you can go back and edit the settings of Form Publisher.
Now you should be able to go back to the Google Form and look at the settings of Form Publisher.
To do this, click on the 3 lines in the top left corner. This will show a list of all the Settings that you can click on and edit to your liking.
