Immunization records must be uploaded to Medicat following the instructions at hofstra.edu/medicatguide. Immunization records can be obtained from former educational institutions, your healthcare provider, and/or your local health department. They should be signed or stamped by the appropriate healthcare provider or be a verifiable report from their electronic medical record (EMR).
All immunization records must be recorded in English. Translated documents must be accompanied by documentation from a professional translation service; translation by the patient, a family member, or friend is not permitted.
Hofstra University policy and New York state regulations require:
(1) Proof of vaccination against or documented immunity to Measles, Mumps, Rubella (MMR) and
(2) Proof of vaccination against meningitis within the last 5 years OR declination of meningitis vaccination. Note that either Meningitis ACWY or Meningitis B vaccine will meet this requirement, but we generally recommend that students receive vaccination against both strains.
Students should contact Student Health with any questions or concerns regarding their immunization status. Many vaccines, including MMR, Meningitis ACWY and Meningitis B, are available through our office or your local health department.
