2023-2024 Spring SC5100 Special Education CCEIS & CEIS


The SC5100 data collection must be submitted annually by LEAs to report the number of children who received coordinated early intervening services (CEIS) and who subsequently received special education and/or related services. 


As described in the Program Guidelines associated with the 2023–2024 Special Education Consolidated Grant (Federal) application, this reporting requirement only applies to any LEA that reserved CEIS funds during the 2021–2022, 2022-2023, and/or 2023-2024 school years, as reported in their respective 2020–2021, 2021-2022, and/or 2023-2024 Special Education Consolidated Grant (Federal) applications.

 

Additionally, LEAs required to reserve CCEIS funds in the 2023–2024 Special Education Consolidated Grant (Federal) application due to significant disproportionality must report the number of students with disabilities served as well as students without disabilities served during the 2023–2024 school year.

 

All LEAs, including single-member LEAs and fiscal agents of shared services arrangements (SSAs), who reserved CEIS or CCEIS funds during the applicable years must certify and submit this data by the deadline date of May 31, 2024.

LEAs may access the SC5100 CCEIS/CEIS Data Collection form through the TEAL platform.

Note: For fiscal agents of SSAs, the SC5100 data must be reported at the member LEA level.  A drop-down box is available on the form for the fiscal agent to select the specific member LEA(s) in the SSA that reserved CEIS or CCEIS funds.
