Please read through pages 1,2 and 3 of the linked document. Use the page 2 template to create a design and complete the entry details, including the 'Teacher name' as Mr. Hudson, 'Teacher email' as aubrey.hudson@education.vic.gov.au, and the 'School number' as 98074944.

Once you have done this, scan and email 'page 2' to ride2school@bicyclenetwork.com.au by the closing date of Tuesday, 30th November.
