COE Form

This is VERY important for you to use when I send you students who are being reported by our district and another CO district.

• If the student is active in your program, you will need to send the COE form to the other district.

• If the student is not active in your program, you will need to inactivate the student in Synergy with the date they were last active in your program and exit code 13 (since they are being claimed in another district).

• You will also be asked to fill out COE forms and return to other districts as well to confirm enrollment in your program.

• If a student is active in your program and active in the other district, the parent will need to be called to unenroll in one of the programs. 

• The COE form needs to be sent with your School name on it.
