Assessment Opt-out form 

Please begin collecting assessment opt-outs and send them to Ashley via dropbox as soon as you receive them. If you are using the BOCES opt-out form, Ashley will update you when those come in. Currently, there are NO opt-outs submitted using the BOCES form. Additionally, please upload a simple excel document to dropbox stating the student's name, grade and the assessment(s) they are opting out of as well as the signed forms. 

It is also the SAC's responsibility to code the student as "parent excuse" in their profile in Pearson for each assessment they opted out of. The opt-out list submitted to Ashley is for compliance purposes and the ones missed can be submitted during the SBD process BUT the schools are responsible for coding the students in Pearson as they come in.
