This form is for non-emergency items and you should respect a response or a confirmation that we have received your form within 48 hours.
In the case of an emergency, you should reach out directly to your leadership staff or the urecmembserv.edu email.
The goal of this form is to talk about your questions or concern within your next check-in, so you and the leadership team can help figure out the best solution for you to feel comfortable in the job.
You can also look at this form as a suggestion box about how you feel about certain policies/procedures we have put into place.
This form is anonymous, so please tell us how you truly feel. We want to hear your voice and we want you to know that we take your opinions and feelings seriously.
You may add your name if you want us to follow up with you, but this is not required.
