Each participant must complete a waiver. This is for the SRC and Quinn.
Approval from the Office of General Counsel and required for participation.
Valid for a 5 year period.
Covers participation in facilities, IM Sports, Climbing Wall, Trips, and Group Fitness classes.
Front Desk staff are responsible for ensuring it is completed.
Fusion will show the designated “membership” once the waiver has been completed.
If someone is less than 18 years of age, the parent/guardian must complete the waiver for them. Important to remember for family memberships.
They can bring a signed copy but make sure it’s signed (not typed).
Our waiver forms are blue.
Facility Access [Member Services & Climbing Wall will complete these steps]
Individual approaches front desk to sign waiver
If they have completed the waiver, when they swipe for Facility Access it will show the “membership” for Waiver - General UREC
If they have not completed a Waiver, it will not be indicated when they swipe in for Facility Access
Member Services and Climbing Wall staff should ask for them to complete the waiver
Can indicate in Fusion that a Waiver has been completed by giving them the Waiver - General UREC Membership
Set the date for 5 years from completed date

Member Services/Climbing Wall:
Checking for legibility of name (can someone read it to be able to identify who the waiver is for)
At closing time of the facility, Completed Waivers need to go to designated location at the front desk
Importance of Waivers!
It is extremely important that when people swipe in that they have a wavier!
This means if you are checking out equipment, you should switch back to the Live access folder to ensure the swipe goes through and the person has a wavier.
If the swipe is slow make sure to keep the person in front of you and wait till the person comes up on the screen (to ensure there is a wavier)
Stop your conversations and make sure you are facing the computer
this allows you to focus on the screen and make sure everyone has a wavier
If there are 2 member services on shift one should come out behind the desk and stand by the swiper
If someone does not have a wavier, it will be easier to stop them
What to do if a person slips by without a wavier
Try to grab their attention before they get to far away, say their name, say excuse me, etc.
Use the PA system to ask the person to come down to the front desk
If the supervisor isn't doing anything ask them to catch the person/go find the person in the facility (they can bring a wavier with them)
If there are 2 member services on shift, have one go grab the person
If someone enters the facility without a wavier and then gets hurt, YOU then become liable for the fault
meaning you can be named in a lawsuit if they file one, etc.
We don't want this happening!!! This is why waivers are so important
