What is an Incident Report? This is a form used to document certain events that happen within the facility. In most cases, the Facility Supervisor will fill this out, but you need to know about it as well. These events include:
When Outside & Help has to be Called (EMS, ASU Police)
AppCard Misuse/Facility Entry Concerns
Minor and Major Facility Issues (Ceiling Leaks, Equipment Damage, etc.)
Locker or Lock Incidents
Thefts
Anything Else that Seems Out of the Ordinary
Things to Keep in Mind:
Where Can I Find an Incident Report? These are located in the file cabinet in the bottom drawer next to the Front Desk computers at both the SRC and Quinn. If you can't find any there, you can check in the main office at the same place where you find the extra waivers (next to the walkie-talkies). If you're still having a hard time finding some, you can always call and ask for help!
Where Do I Put the Completed Incident Reports? These can be given to either Angela Shook or Eddie Simmons. If neither of them are in the office, you can slip them under their office doors (Angela's is in the main office and Eddie's is back where the Outdoor Programs offices are). If you are at Quinn, please take a picture of both sides and submit with your shift report and then place it in the completed waivers stack.
Please Make Sure to be as Specific as Possible When Writing About the Incident!
Please Make Sure You Fill Out all of the Information on the Incident Report, Including getting Signatures from the Participant and First Responders!
