Surgery Records (Sheets)
Surgical Records (Sheets) that is 1. surgery record sheet detailing the surgery as well as 2. the recovery sheets for post surgery monitoring must be kept by the lab for at least 3 years after the death of the animal. Make sure you know where and how to locate the surgery sheets for an animal you have done surgery on in case the vet or BIACUC requests to view it.
Non-Invasive "surgeries" (anesthetizing the animal)
Occasionally additional non invasive "surgeries" or periods where the animal needs to be anesthetized are done. For example drilling away extra acrylic from the surgery site to stop scratching. Or replacing gold pins that have fallen out of the EIB. In any case where the the animal is anesthetized however a surgery record sheet needs to be filled out and completed just as you normally would, however you do not have to fill out an additional recovery sheet.

The surgery record sheet and recovery sheets are attached below. If you are uncertain please check that you are using the most updated versions.
The surgery record is a modified version from the LAR website with vet approval ,and the recovery sheets are embedded in the labs protocol.
Where to keep the sheets
While the animal is actively recovering from surgery the surgery sheets should be kept near or directly under the animals cage.
After the animal has successfully recovered from surgery the surgery sheets should be kept in the blue expanding folder underneath the cart in the colony room.
After the animal has been sacrificed the surgery sheets for that animal should be moved out of the blue expanding folder and placed in the purple expanding folder located in 483 (for three years).
Filling out the sheets
It is important that you adequately fill out the surgery sheets appropriately and includes all the relevant information, and that the animals are monitored and checked for the appropriate number of days post surgery during recovery. If you have any questions be sure to ask Ehren or any senior member of the lab.
Attached below is an example of an adequately filled out surgery record sheet. In addition all surgery records should be posted to the #surgeries slack channel post surgery for Ehren's approval.
