Form 1095

Starting with the 2024 tax year (reported in 2025 and beyond), employers are not required to automatically mail Form 1095-C to all employees.  However, if you still wish to receive a copy for your own records, you can request one by completing this FORM. Please request a form by emailing submitting this  benefits@HHH.k12.ny.us   Forms will be mailed to the address on file.
