Form 1095

Starting with the 2024 tax year (reported in 2025 and beyond), employers are not required to automatically mail Form 1095-C to all employees.  

If you still wish to receive a copy for your own records, you can:

• request one by completing this FORM

• request a form by emailing  benefits@HHH.k12.ny.us  

• request a form by calling us at 631-592-3094

• request a form by mail to: Benefits Department at Half Hollow Hills CSD 525 Half Hollow Rd, Dix Hills, NY 11746   

Regardless of your request method, the printed Form 1095 will be mailed to your current address on file with the District.
