Benefits vary by contract and are fully paid for by the district. Below is a summary of where to submit which application when you are hired. If you have changes to your beneficiaries, you can make that change at any time.

The Change of Beneficiary Form for Mutual of Omaha can be downloaded and submitted to Employee Benefits via email or interoffice mail.

ADMINISTRATORS - 2x salary through Mutual of Omaha - submit application to Employee Benefits via email or interoffice mail

CLERICAL - $10,000 through Mutual of Omaha - submit application to Employee Benefits via email or interoffice mail

INTERNATIONAL BROTHERHOOD OF TEAMSTERS - $10,000 through Mutual of Omaha - submit application to Employee Benefits via email or interoffice mail

MAGCON and SUPERINTENDENTS - see individual contract for amount. Through Mutual of Omaha - submit application to Employee Benefits via email or interoffice mail

TRANSIT UNION WORKERS - $7,000 through Mutual of Omaha - submit application to Employee Benefits via email or interoffice mail

TEACHERS - 2x salary through ASO - contact office@hhhta.com for more information . Complete and submit the HHHTA Beneficiary Form to HHHTA for processing.

ALL OTHER EMPLOYEES - The district does not administer nor offer life insurance. Contact your union representative to see if there are options available to you.

NOTE: For Group Term Life Insurance amounts over $50,000, employees will be taxed in June and December for this fringe benefit. The "N/C LIFE" amount shown is based on IRS Publication 15-b. More information can be found on the IRS Website here.
