What to do for certain qualifying events in NYSHIP - ( for HIP changes, use THIS FORM)

Getting Married

TO ADD YOUR SPOUSE

    a. Complete the Transaction Form to [A]dd your spouse to your existing policy and/or Change your policy from Individual to Family

    b. Attach required copy of your marriage certificate and spouse's birth certificate

    c. Sign and date Form and submit paperwork to HHH Benefits Department at Central Office within 30 days of the marriage

TO CANCEL YOUR POLICY

    a. Complete the Transaction Form to Cancel your current policy with HHH

    b. Attach a copy of your marriage certificate to document the qualifying event

    c. Sign and date Form and submit paperwork to HHH Benefits Department at Central Office within 30 days of the marriage
