Self-nomination forms are due back in the office by 3 p.m. on Firday, March 13th

This is a strict deadline; the committee will not consider late applications.

Application Form instructions

step 1: select the google doc below for the NHSapplication (doc is below)

step 2: make your own copy and save document under a new name in your Google Drive

step 3: complete application by typing in your information

step 4: print and hand in your application to the office
