About the Program

The Street Medicine service is a pilot program that takes medical care services of HOPE to the people on the street. Individuals experiencing substance use disorders can be connected with other agency providers who can mitigate detox/rehab treatment using HOPE Services as an entry for referrals.

MEDICAL INTERVENTION:

  • Wound care

  • Prescription delivery

  • Connection to shelter and housing services

  • Medication cost assistance

  • Telemedicine

  • Help with receiving insurance
