Three-Year Degree Information
Assessment form
The Three-Year Degree Assessment Form is to help the student decide if switching to the Three-Year degree is the right choice. The Mentor will be meeting with the student to help you navigate through the Three-Year Degree Assessment Form.

After completing this form, the student will either:
● Stay in your current degree program here at BYU-Pathway Worldwide
● Switch to BYU-Idaho Online to keep Federal Financial Aid
● Select to speak to an Advisor to learn more
● Switch to the new Three-Year degree (will require you to meet with an Advisor)

Ticket
When students fill out the form with their mentors or request their mentors to switch to the three-year degree, these surveys will be sent to advising as tickets in they will be placed into the Three-Year degree queue.
When selecting the ticket, please pay attention to the following:
● The Subject of the ticket
● Survey response
● Escalation description information
The information provided in the survey will help you identify the student's interests and
concerns.

After reading the ticket information, the next step is making the analysis :

Email & Template
Once we obtained the result of the Power app tool and the analysis using the KB’S. You will have to send an email explaining the changes to the student and ask for confirmation about his/her decision.
- Be clear on your explanations
- Explain that more changes can come in the 2024 catalog
- Ask for written confirmation

Ticket processing and submission to the DPT
Create a ticket or escalate the original one to the degree planner team and attach a screenshot of the student’s confirmation, add this information to the notes tab of the ticket.
Make sure to include precise and objective information, please pay attention to the following:
● Fill out the topic and each field correctly
● In the Escalation description enter detailed information about the current and new student’s degree, catalog year, and certificates.
● In the ticket’s status, you must select “Your ticket is created and waiting on an agent”
● Click on Save and obtain the ticket number to include in the 3-year degree spreadsheet.
● Don’t forget to send the ticket to the Degree Planner Team.
Escalation description information:
Once you created the ticket and submitted it to the Degree Planner, make sure to include the ticket number and student’s information in the Students Needing UG24 Catalog Change spreadsheet.
BYU-Idaho students who wish to participate/enroll in the Three-Year degree will need to have their catalog year changed to the 2023 catalog year if they are not already on that catalog, and then later to the 2024 catalog year when it is released. Ensign College students will remain on their current catalog years until the 2024 catalog is released, and then their catalog will be updated.
For students who are already in the 2023 Catalog Year and request to switch to a three-year degree program, The advisor will need to do all the procedures with the ticket except to escalate it to the Degree Planner Team. The advisor will have to complete the ticket.
Sub-Program Changes
PWOL Students who currently receive OR plan to receive financial aid will need to have their sub-program updated to ONLN.
BYU Idaho Students
After pulling the ticket and confirming with the student that they wish to opt out of the Three-Year Degree and be moved to Idaho in order to remain Financial Aid eligible, open and fill out this spreadsheet: BYU-Idaho Sub-Program Change Requests
Add the following information to the spreadsheet:
1. Student's name
2. Student I-Number (not the PATH ID, but the BYU-Idaho I-Number)
3. The student's current catalog year
4. The student's current degree
5. Advisor’s name
6. The student’s ticket number
7. DO NOT fill in the “Date Sent to Idaho” column. That is for the Registration Degree Specialist to track the records they send.
After filling out the spreadsheet, you should contact the student to let them know they have been added to a list for BYU-Idaho to move them to their system and that they will be contacted by someone from the BYU-Idaho advising center with further instructions.
After you have contacted the student letting them know the above information, you can close and mark the ticket complete.
Ensign Students
If the student needs to have their sub-program changed to EC, the Advisor can request this change by filling out the PWOL to EC Form with the following information:
1. Student’s First Name
2. Student's Last Name
3. Ensign Student ID#
4. Student Email Address
Check the box that says “Send me a copy of my responses” and enter your Advising byupathway work email address in the box that appears.
After the form has been filled out and submitted, the student can expect Ensign College to notify them of their subprogram change once the request has been completed.
Once the form has been submitted and the ticket has been filled in with all the required information, the Advisor may then close and mark the ticket complete.
