risk to others - threaten to kill neighbiurs

relationship with family members/ partners / colleagues and neighbours 

Past medical history / Past medication history / history of admission to ward / history of going respite / history of previous suicidal attempt 

Alcohol / substance use history 

tolerance ? 

dependency - history of trying to stop - methods used - description of experience of trial to stop the substance
