Approach to thrombocytopenia 

Pseudothrombocytopenia ? platelet clump - fbc is usually send in EDTA tube - send in citrate tube when observed platelet clump in the purple tube

PBF 

- schiscocytes ( fragmented blood cells-  ITP/ TTP ) 

- promyelocytes ( APML ) 


Pregnant thrombocytopenia 


Pancytopenia --> bone marrow 


TTP - platelet transfusion is contraindicated 

ITP - do not routinely transfuse even if they have bleeding - swamped by autoantibodies - only transfuse when they going for surgery / massive hemorrhage  - treat with steroids
