Endocrine emergencies 

Hypoglycemia 

less than 1 , 1-2 - dextrose 50% 50 ml 

repeat in 15-20 mins 

dextrose 10% over 8 hours 

DKA 

diabetes BGL 25  

ph < 7.3 

Ketone : more than 3 

normal saline 

1 litre in 1 hour 

1 litre in 2 hour 

1 litre in 4 hour 

1 litre in 6 hour 

Insulin 0.1 u / kg - 6 unit / hour 

 hourly blood glucose - until glucose less than 25 

give normal saline 0.45% 

from second bag of normal saline on ward 

potassium < 5 - replace 10mmol in each 1 litre of normal saline 

aim ketone < 0.6
