Guidance on Collecting Statements from Educators and Students:
Statements from educators involved in testing irregularities
Obtain separate, typed, signed, and dated statements from all individuals who were involved in or may have information about the incident.
Verify that each KTX Irregularity Report Form statement includes:
name, title, contact information, and role during testing;
how individual was responsible for or involved in incident;
description of incident from individual’s perspective;
individual’s signature; and
date the statement was signed.
Individuals implicated in a serious violation should be informed of the allegations being made and asked to respond directly to all accusations.
Statements from students involved in testing irregularities
Obtain statements from any students who were involved in or may have information about the incident.
Students' statements may be handwritten or recorded and transcribed.
If the district records a student’s statement (video or audio), written consent from the student’s parent must be obtained as required under the TEC §26.009.
Contact information for each student should be maintained at the local level in case TEA requests it for a hearing.
CTCs - please use this form to add missing students to Cambium, and to report students who need to be removed from Cambium. Student information should be verified in Skyward or from the Test Hound Student Demographics page (click on student list, click on demographics at the top). We are unable to add students with incomplete information.
Missing students - Students who need to be added will be added within 48 hours if all information is provided
Withdrawn students - students will not be withdrawn until the end of the testing window to clean up your data. Districts are able to transfer students to their new school in Cambium so leaving them there makes it easier for new districts to add them.

Requests that are not updated with missing information 48 hours after the request will be marked red and CTCs will need to resubmit.

The Texas Education Agency requires test security training for all staff who will actively involved and/or physically present on a KIPP Texas Campus during STAAR, STAAR ALT2, TELPAS, TELPAS ALT, and official Cambium IAs regardless of whether or not the employee directly interacts with testing. In addition, any KIPP Texas Public Schools employee who needs to access secure test data and student results must also be trained in test security measures. Test administrator training on test security procedures is required for all employees at least once a year. At KIPP Texas we highlight and remind staff about test security at every training.
For the 2024-2025 school year, the process for completing test security training, oaths of test security, and signing up for a Cambium account (access to the centralized reporting system CRS, TFAR, DEI, TIDE, etc.) will be similar to last year.
All KTX employees, including those who will be on campus during active testing or who will need access to student testing data must complete the mandatory asynchronous test security training through cornerstone. The 2024-2025 Cornerstone Test Security Training for All KIPP Texas Employees will be pushed out by EOD today, 9/16/2024. Anyone who needs Cambium access for Equity Data Day on 9/20/2024 will need to complete the three steps listed above by end of day Thursday 9/19/2024.
Test Security Training and Oath - Will be released in September 2024 through Cornerstone. It is required for all KTX Staff. At the end of the training, the oath must be signed.
Cambium Access - All staff who wish to have a Cambium account for the 2024-2025 school year must complete all three steps listed below:
Complete the 2024-2025 Cornerstone Test Security Training Module released to all KTX employees on 9/16/2024
Please note - your training will not be marked complete in Cornerstone if you do not score 80% or higher on all quizzes (you can return to them and redo them if needed) and/or if you do not scroll all the way to the very end of the final training screen.
If the training is not automatically assigned to you, you may search for it in Cornerstone. In the top right corner click on the search icon and type in "test security". Please make sure you select this year's training not the 23-24 training.
Sign the Test Security Oath. Directions are in the Test Security Oath section of the Cornerstone Test Security training. (Oath Rollup)
Make sure you spell your KIPP Texas email address correctly on this form.
Non-certified staff must know the name of the certified staff member who will supervise them before filling out the oath. If you are not certified in Texas, you will not be eligible for a Cambium account if you do not name a certified supervisor on your oath. You will be asked to redo your oath with the correct information.

There are no rollover accounts. All KTX personnel who need access to Cambium must reapply for a new account every year.
Once an account has been activated, please use these instructions on how to sign into your account for the first time each school year.
** Cambium accounts will not be activated until all 3 parts are completed and verified.
If you had access last year, you will continue to have access until August 16th of this school year. The Cambium account blackout and rollover dates will be included in the KTX Testing Strategy and Assessment Calendar.
After this date your account will be deactivated and removed from Cambium.

Irregularities happen! Document them and call your RAC!
Making sure you follow accommodation procedures is extremely important for STAAR. If accommodations are not correct, the student’s test results and your campus’ accountability may be negatively affected.
When it comes to STAAR testing, if it is on the IEP/IAP/EB testing page - we must provide it, if it is not - we can’t provide it.
No manual changes to testhound everything has to come through official channels (ESPED/Ellevation).
You MUST meet IN-PERSON with your Special pops coordinators prior to testing. You must have signed documentation of this meeting.
You must upload and verify all testing attributes by the deadline.
All campus staff (you included) must complete the Test Security Training and sign the oath by EOD on 11/22!
No one can access STAAR materials without being trained and without signing the official TEA Oath. This includes parent volunteers, SSP volunteers, etc.
Knowingly allowing staff members to be involved in testing without completing TS training and the oath is a serious Irregularity.
CTCs are responsible for tracking Training/Oath completion for all grownups present on your campus.
You must train your staff on STAAR testing logistics, procedures, and test security. Use the Turnkey Training Slide Deck . Make a copy and update it for your campus.
You must follow the District Testing Dates plan for STAAR assigned testing dates.
Practice Tests are Required. Students must practice using the tools prior to testing.
When in doubt, reach out to your RAC! There are no “dumb” questions when it comes to testing! You’ve got this!
