Submitting a Name Change to DFAS for the Retired & Casualty Pay System (RCPS)

Submit your record correction request in a letter by including your Social Security number, date of retirement and signature along with the original or a certified copy of the legal document recording the change. Also include a copy of a current SSA card, driver’s license, and if available, passport, all showing the name change. Send to:

You must provide in the email a signed memo with the following statement:
“I acknowledge and certify that the attached document is a true, exact, complete, and unaltered electronic copy of the original/certified copy” and include the following documentation:
1. Court order, marriage certificate/divorce decree, copy SSA card, copy driver’s license, and copy of passport if available, showing the name change.
2. State both names as reflected on legal documentation (previous name and new name)
3. Contact information including mailing address, email address and phone number
4. INCLUDE IN THE MEMO THE FOLLOWING STATEMENT “I acknowledge and certify that the attached document is a true, exact, complete, and unaltered electronic copy of the original/certified copy.”
WHAT HAPPENS NEXT
Once the correction action is completed by the ARMS Branch, an Air Force Form 281 (Notification of Change in Service Member’s Official Records) is filed in your military record. The original or certified copy document recording the correction action and a copy of the Air Force Form 281 will be provided to you for your records. A copy of the Air Force Form 281 will be forwarded to the Defense Finance and Accounting Service as well for its record update. DFAS sends the change confirmation to the Defense Manpower Data Center for update to your record in the Defense Enrollment Eligibility Reporting System.
You may also send a copy of your legal document supporting the correction in advance to the DMDC Beneficiary Helpdesk by calling 1-800-538-9552 (Fax: 831-655-8137), pending confirmation from DFAS.
NOTES:
If the request is based on a change by court order, or to comply with local state law, the original or certified copy (not a notarized copy) of the court order or decree or evidence of compliance with state law regarding changes of name in state where individual is located is required. If the name change is due to naturalization, the original naturalization certificate or Department of Justice statement showing correct name is required.
(The Air Force does not correct records of former members unless evidence proves the name used while serving with the Air Force was erroneously recorded.)
