JotForm is a versatile form/survey platform that allows us to build all sorts of forms. We can integrate payments and workflows (approvals) into forms; as well as generate nice looking PDF's from submitted data.
JotForm is used by several teams at the Shared Services Center - some notable examples:
SSC Accounts Receivable Payment Form

(Several teams) Audit forms (internal audit processes)
Tech Team has used it to capture Cybersecurity 101 Feedback (on the videos) and to survey SSC Teams about applications and software that they use
SSC Performance Evaluation forms (NEW in 2025!)
Submit a ticket to the Tech Management Team - with your idea - and we will work with you to build out a form and the backend structure. We can send results to Google Sheets, Dropbox, or have BAT integrate it into a Tableau report!
We wouldn't recommend it. The SSC has an Enterprise contract - but the Enterprise licenses are limited to the Tech Team.
Free accounts have restrictions. There are also privacy concerns, depending on what type of data you are collecting. See the brief list (at right) on some of these restrictions.
