The Mathematics Department is pleased to support academic programming. Please email mathgradadvising@ucsc.edu before making purchases - it is important to seek approval prior to spending to ensure that funds are available and the expenses are eligible for reimbursement.
Students making approved purchases on behalf of their organization or program need access to CruzFly for reimbursements. Please follow these steps to set up an account:
1). Complete the 204 Form.
Select the first option, "Establish a New Payee with UCSC or Update an Existing Payee"
Section 3: If you are a graduate student, select "Reimbursement", "Travel", and "Prizes/Awards". If you are an undergraduate student, select "Reimbursement" and "Travel".
Put the graduate advisor as the department contact, using their CruzID (NOT the math advising email address).
2). Complete the Access to Information Statement.
3). Once the 204 Form and Access to Information Statements are complete, complete the CruzFly Account Authorization Form.
Since this is a student account, you will need to include a delegate. Under Delegate Information, use the graduate advisor as the delegate.
Under Supervisor Information, select "My Delegate is also the supervisor"
Under Divisional Access Authorizer, enter Meg Lehr (mlehr@ucsc.edu) as the CruzFly Divisional Access Authorizer.
Under Primary FOAPAL Organization Code, enter "403135"
Once you have completed these items, the graduate advisor should receive a CruzFly DocuSign notification to sign off on your account creation request. Once your account is created, you receive instructions on how to get started creating expense reports.
Always touch base with the graduate advisor while completing expense reports. Never submit an expense report. Notify the graduate advisor via email that the report is ready for review. They will be able to access your account, review the report, and submit it on your behalf.
