HR-008 Veteran Self Identification - Required by US Dept of Labor - New Hire completes to voluntarily answer questions in regards to being a veteran
HR-014 Direct Deposit Authorization - Use to key DD info into ERP and retain in Employee records
HR -050 New Employee Information - Internal document that is used to gather basic new hire information into ERP to set up profile
HR-059 Salary Deduction - Needed to reduce an employee's paycheck amount due to being paid for a special purpose; a way of paying TSTC back
HR-060 Driver Authorization - Employee authorization to check driver status in the state to be approved to utilize company fleet
HR-061 TSTC Provided Uniform Form - Used in departments where uniforms are distributed
HR-072A State Employment Verification or Transfer (Page 1 completed by Employee) - Used for transfers from other State agencies to complete transfer of leaves, TRS/ORP, ERS, etc (This page for Employee)
HR-079 Transcript Acknowledgement - Required by SACSCOC; given to new hire who has been hired into a position which requires credentialing
HR-089 Selective Service Eligibility and Verification - Almost all men age 18-25 who are U.S. citizens or are immigrants living in the U.S. are required to be registered with Selective Service. U.S. law calls for citizens to register within 30 days of turning 18 and immigrants to register within 30 days of arriving in the U.S.
HR-092 Voluntary Self-Identification - Required by the US Dept of Labor; employee voluntarily complete as part of new hire paperwork
HR-094 Voluntary Self-Identification of Disability - Required by the US Dept of Labor; employee voluntarily complete as part of new hire paperwork
HR-096 Public Access Option Crime Victim Identification Election - The Public Information Act allows employees, public officials and former employees and officials to elect whether to keep certain information about them confidential.
DOL Notice - Required-TWC/Health Insurance Marketplace
Notice of Workers Comp Coverage - Required-TWC/notice of workers' compensation coverage - whether the company carries workers' compensation insurance or not, it must notify new hires one way or the other
Consent for Background Check - Required-TWC

HR-001 Application to Contribute Time to the Sick Leave Pool - Tier 2 or Leave Admin's use this to process Sick Leave Pool hours
HR-002 Request for Withdrawal of Sick Leave Pool Hours - Tier 2 or Leave Admin's use this to process Sick Leave Pool hours
HR-103 Employee Accommodation Request - Internal document used for an employee to complete who is requesting an accommodation to their work schedule/duties
HR-104 Employee Accommodation Medical Certification (with or w/o physician) - Internal document used to request medical information for a specific case. There are two versions of the workflow, one requires the physician’s signature, the other does not.
HR-017 Prior Optional Retirement Program (ORP) Participation Acknowledgement - Required by THECB - New Hire who previously participated in ORP
HR-018 Optional Retirement Plan (ORP) New Participant/Change of Provider - Required by THECB - New Hire or current employee who are enrolling in ORP or changing provider
HR-020 Optional Retirement Plan (ORP)/Tax Deferred Annuity (TDA) Vesting Status - Required by THECB - Separating/Retiring employee completes with HR; The original form must be mailed to the ORP vendor; a copy must be given to separating employee for their records and a copy must be kept in employee file

HR-027 Request for Release Time and/or Scholarship for TSTC Courses - Use this form to request release time or to request a scholarship for attending a TSTC course.
HR-039 Employee Educational Benefits Program Dependent Certification - Required - SOS HR 2.3.18 Texas Govt Code, Ch 656, Section656.044 Texas Human Resources Management Statutes 2018-201. This is a benefit for employees so that their qualifying dependents can enroll for classes at discounted tuition rate.
HR-091 Update Personal Information - Internal document that an employee completes requesting and authorizing changes to be made in ERP profile
HR-093 Bi-Weekly Timesheet - Required by CPA- In the event that a Biweekly employee misses the cutoff to key hours in WebAdvisor, this sheet must be completed to be paid; it is also used for hours worked during the holidays for students.
HR-067 Manual ACH Request ProtoType 2014 - (ACH is Automated Clearing House) internal document used to process off cycle deposits to employee accounts
HR-072B State Employment Verification or Transfer (Page 2 for Previous Employers) - Used for transfers from other State agencies to complete transfer of leaves, TRS/ORP, ERS, etc (This page for former agency)
HR-105 Retiree Attendance Leave Pay Report - Retirees are sometimes paid on a monthly basis, but do not have a set schedule, keeping them from using the timesheet system within LRS
HR-016 Optional Retirement Program (ORP) Acknowledgement - Required by THECB - New Hire or current employee acknowledgement that they are eligible for ORP
HR-097 Employee Acknowledgement - This document is sent to each new employee, used to confirm they understand SOS and the Faculty/Staff Handbook.
HR-101 New Hire Offer Approval - Internal document used to request position approval
HR-102 Position Request Waiver - (with or w/o SL) Internal document used during a hiring freeze requesting the approval to post a position There is a separate workflow for Student Learning positions requiring an additional signature.
