In the EpiTrax database, Person Records are classified as a CMR (Confidential Morbidity Report), or Contact (non-CMR record). If a
Contact is suspected of or diagnosed with a Reportable Condition (Morbidity), it becomes a CMR.
A CMR generally follows this workflow, although there are additional workflow statuses:
Assigned to LPHA
Accepted by LPHA
Assigned to investigator
Under investigation
Investigation complete
Approved by LPHA
Closed
The Workflow Status is displayed in many locations throughout the program and are color-coded.
Grey- Accepted by LPHA, Investigation Complete, Approved by LPHA, Closed.
Green- The CMR is under investigation.
Pink- The CMR needs acceptance, as it is either currently assigned to LPHA or assigned to an investigator.
There are multiple ways to locate CMR and Non-CMR (Contact) Person Records within EpiTrax. Most searches have a Star Rating indicating the accuracy of
each match. Star Ratings are determined by how many name field values match a record. This means even highly accurate results will not always get five stars.
The EpiTrax Surveillance System is a person-centric application. Each person in EpiTrax is considered a Person Record. A Confidential Medical Record (CMR)
is an Event Record where that person is suspected to have or diagnosed with a Reportable Condition (Morbidity). Contacts are Event Records associated with a
Morbidity report in some way, (parent or guardian of a minor, person exposed, etc.). Contacts suspected to be infected with a reportable condition can become CMRs.
By clicking the NEW CMR option from the EpiTrax Application Menu, users can view and navigate the New CMR screen. This screen is designed to prevent the creation of duplicate CMR records, by having the user first search for matching records before the buttons to Create a New CMR appear.
For events requiring investigation - for both lab confirmed and unconfirmed diagnosis.
The CMR is the form used to report most diseases and other conditions in Public Health. In EpiTrax, each CMR indicates a patient record, to which various data can be connected.
IMPORTANT: To avoid duplicate CMR records, ALWAYS search for an existing patient CMR in EpiTrax first, before creating a new one.
Click the NEW CMR link in the Application Menu of EpiTrax.
If no option for NEW CMR, ensure you are logged into EpiTrax, and click the EpiTrax logo (upper left) to navigate from the home screen.
Click Search to check for existing records of patient data
You do not have to fill in all fields to perform a search. If data is entered incorrectly in any field, it will be outlined red after clicking Search.
Click + Create Person and CMR if no matching results are found
If the initial CMR Search returns a match, DO NOT CREATE A NEW CMR.
Enter a valid date into the First reported to the public health field
Click calendar or enter date values in this format: mm/dd/yyyy
EXAMPLE: 01/01/2001.
Select Disease
Select Investigating Agency
Click the desired Save button to finish saving the CMR
Save & Continue saves the record but stays in it (remains locked).
Save & Exit ensures that all work is saved and the record is unlocked.
