HIPAA must be signed once a year.
Keep the explanation short - "This is our HIPAA privacy form, the first three sections talk about how we use your information, the last section states if your insurance does not pay for the exam, you are financially responsible for the cost of the exam".
The HIPAA form actually acknowledges the patient had an opportunity to review our "Notice of Privacy Practices". If they ask to review it, print a copy and give to patient. The file is located under the Forms Tab.
If patient refuses to sign, they can still have exam, but make a note in Crystal under Note Tab (Personal Notes). If they want to use insurance, they must sign for at least the bottom 2 sections, otherwise they can be seen but will be a Self Pay.
